This site implies that you do: http://www.dominicanembassy.org.uk/Touristcard.html. I have read that you can buy it on arrival for $20pp.

Someone on cruise critic advised "Cruise line passengers are regarded as being in transit and don't require a tourist card. If you are staying in Santo Domingo I think you will need one.Don't worry if you don't get one on disembarkation because you will be able to get one at the airport. They check if you have one at the airport. When we arrived at the airport the coaches for the transfer to the ship came to the aeroplane and all cruise line passengers got on the coach and were whisked to the ship without having to enter the terminal. After the cruise we checked in at the dockside and got our boarding cards. So that when we arrived at the airport we we were put in a special line that just went to the departure lounge and didn't go through he check for a tourist card." .
